Applicant secures application form and file the same
to the DTI Provincial Office where the business is
located.
2.
Pay a registration processing fee of PhP 121.00 and
submit the following documents:

Form No. 17 under R.A. 7042

appointment of Filipino Resident Agent

Authority to verify

remittance of foreign currency for non-resident
alien and Bank Certificate of Deposit for resident
alien

Copy of valuation
report from Central Bank if investment includes
assets other than foreign exchange

Clearance from other
involved agencies such as Department of Science
and Technology, Philippine National Police, etc.

Proof of citizenship
if applicant acquired Filipino Citizenship by
naturalization, election or by other means
-
Proof of citizenship
(e.g. Birth Certificate, Voter’s ID, etc.) if
Filipino name is suggestive of alien nationality

Applicant who maintains branches must
register each and every branch separately.
4.
Applicant shall be issued a Certificate of
Registration of Business Name which shall be valid for
five (5) years.
